Liability Policy

                            AZ Soccer Training Liability Policy

I, the undersigned, acknowledge that participation in private soccer lessons involves inherent risks, including but not limited to physical injury, equipment malfunction, and accidents. I understand that these risks may arise from my own actions, the actions of others, or the nature of the activity itself.

                                       Waiver and Release

In consideration for being allowed to participate in the private soccer lessons, I hereby waive, release, and discharge AZ Soccer Training, its owners, employees, instructors, agents, and representatives from any and all claims, demands, or causes of action that I may have for any injury, loss, or damage arising out of or in connection with my participation in these lessons, whether caused by the negligence of the released parties or otherwise.

                                          Indemnification

I agree to indemnify and hold harmless AZ Soccer Training from any claims brought by third parties as a result of my participation in the lessons.

                                       Medical Treatment

I understand that in the event of an emergency, [Business Name] may take appropriate action to seek medical attention. I consent to any medical treatment deemed necessary.

                            Acknowledgment of Understanding

By booking an appointment, I acknowledge I have read this policy and release in its entirety, understand the terms, and voluntarily agree to the stated conditions.
